| (1) Every application for leave to appeal against an order of court made in the course of any civil action, proceeding or matter, shall be made by the party aggrieved or his registered attorney, shall be supported by an affidavit, shall be in the form, shall contain the particulars, prescribed by rules of court, shall bear a stamp of the prescribed denomination and shall be filed in the Supreme Court. |

| (2) Upon an application for leave to appeal being filed in the Supreme Court, the Registrar shall number such application and shall, as speedily as possible, submit such application to a Judge in chambers. |
